Register with Gold Street Surgery

We will register anyone living within our practice boundary, which covers an approximately seven mile radius of Saffron Walden (almost 154 square miles). Please ask at reception if you're not sure. Please, if possible, bring your medical card(s) to the surgery when you register. We will also ask you to complete a short form and a medical questionnaire, including details of your last doctor.

If possible, please bring current photo ID (such as a Driving Licence or a Passport) with you to register. For legal reasons, we will need a photo ID to register you for the full range of online services. This is not essential to register with the practice for medical services, however. If you have no fixed address, please discuss with us how we can best register you.

We recognise the important role of carers in looking after dependent relatives, so please tell us if you are a Carer (doesn't have to be for a Gold Street patient).

It is our practice policy to only register children if an appropriate close adult relative (usually their mother or father) is also registered at the surgery.

Your registration will be with the practice but it is the right of patients to express a preference of practitioner. Please do this on the new patient questionnaire. Whilst the practice will endeavour to offer you an appointment with the practitioner of your choice, that might not always be possible. Many GPs choose to work part-time, which means you may have to wait longer for an appointment with a specific doctor.
